Per Hyzaar 50 mg/12.5 mg FC tab Losartan K 50 mg, hydrochlorothiazide 12.5 mg. Per Hyzaar 100 mg/12.5 mg FC tab Losartan K 100 mg, hydrochlorothiazide 12.5 mg. Per Hyzaar DS 100 mg/25 mg FC tab Losartan K 100 mg, hydrochlorothiazide 25 mg
Indications/Uses
HTN in which combination therapy is appropriate. Reduction in the risk of CV morbidity & mortality in hypertensive patients w/ left ventricular hypertrophy.
Dosage/Direction for Use
HTN One 50 mg/12.5 mg tab once daily, may increase to one 100 mg/25 mg tab or two 50 mg/12.5 mg tab once daily. Reduction in risk of CV morbidity & mortality in hypertensive patients w/ left ventricular hypertrophy Initially 50 mg of losartan tab once daily. If goal BP is not reached, may be titrated to one 50 mg/12.5 mg tab & may be increased to one 100 mg/25 mg tab once daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to losartan K, hydrochlorothiazide & other sulfonamide-derived drugs. Patients w/ anuria. Not to be administered w/ aliskiren in diabetic patients.
Special Precautions
Patients w/ bilateral renal artery stenosis or stenosis of the artery to a solitary kidney. Compromised renal function (elderly or vol depleted patients) being treated w/ NSAIDs (including COX-2 inhibitors). Occurrence of hypotension & electrolyte/fluid imbalance, marked hypercalcemia, increased cholesterol & triglycerides level, SLE. Dosage adjustments of antidiabetic agents & insulin may be required. Thiazides should be discontinued before carrying out tests for parathyroid function. Dual blockade of RAAS w/ ACE inhibitor or aliskiren is associated w/ increased risks of hypotension, syncope, hyperkalemia & changes in renal function. Closely monitor BP, renal function & electrolytes of patients on losartan & other agents affecting RAAS. Concomitant use of other drugs that may increase serum K may lead to hyperkalemia. Patients taking hydrochlorothiazide should be informed of the risk of non-melanoma skin cancer & advised to take preventive measures to reduce sun & artificial UVA exposure. Regularly check skin for new lesions & report suspicious skin lesion to their physicians. Patients w/ previous non-melanoma cancer. Hepatic or severe renal impairment. Pregnancy & lactation.
Adverse Reactions
Dizziness, fatigue/asthenia & vertigo. Non-melanoma skin cancer (basal cell carcinoma, squamous cell carcinoma); thrombocytopenia, anemia, aplastic & hemolytic anemia, leukopenia, agranulocytosis; anaphylactic reactions, angioedema including swelling of larynx & glottis; anorexia, hyperglycemia, hyperuricemia, electrolyte imbalance including hyponatremia & hypokalemia; insomnia, restlessness; dysgeusia, headache, migraine, paresthesias; xanthopsia, transient blurred vision; palpitation, tachycardia; dose-related orthostatic effects, necrotizing angiitis (cutaneous vasculitis); cough, nasal congestion, pharyngitis, sinus disorder, URTI, resp distress, pneumonitis, pulmonary edema; dyspepsia, abdominal pain, gastric irritation, cramping, diarrhea, constipation, nausea, vomiting, pancreatitis, sialadenitis; hepatitis, jaundice (intrahepatic cholestatic jaundice); rash, pruritus, purpura (including Henoch-Schoenlein purpura), TEN, urticaria, erythroderma, photosensitivity, cutaneous lupus erythematosus; back pain, muscle cramps & spasm, myalgia, arthralgia; glycosuria, renal dysfunction & failure, interstitial nephritis; erectile dysfunction/impotence; chest pain, edema/swelling, malaise, fever, weakness; liver function abnormalities.
Drug Interactions
May decrease effect w/ NSAIDs including COX-2 inhibitors & grapefruit juice. Losartan: Decreased active metabolite levels w/ rifampicin & fluconazole; increased serum K w/ K-sparing diuretics (eg, spironolactone, triamterene, amiloride), K supplements, salt substitutes containing K; or other drugs that may increase serum K (eg, trimethoprim-containing products); decreased lithium excretion. Hydrochlorothiazide: Concomitant use w/ alcohol, barbiturates, narcotics, antidiabetic agents, other antihypertensives, decreased cholestyramine & colestipol absorption, corticosteroids, ACTH, glycyrrhizin, pressor amines (eg, adrenaline), skeletal muscle relaxants (nondepolarizing), lithium. Do not co-administer w/ aliskiren.
